I can't recover my homepage after "apply a form" on it. First, I tried /System/Form/ApplyForm?, then change it to a MemberInfoForm?. Now I got error for both WikiHome page and MemberInfoForm? page. I tried to edit the older version (edit=1&revision=6) and save it but nothing happened. The error looks like the following,

Error: EmptyForm?

Additional information: form MemberInfoForm? returned empty content while displaying WikiHome

I am thinking to try deleting these two pages, but I am afraid of making things even worst. This is a time I will appreciate to have a source code or error code documentation.

Is there any document on how to apply a form on a page? Is there any document on how to remove a form on a page after applying it?
