Added Feedback Form tonight; Basic functions work. When I go to add a comment to a feedback item, I get the following error:

Error: You have specified a form page that doesn't exist.
Additional information: form not found:
/System/Templates/single_tab/AddPageCommentForm?
Please choose a form that exists.

And then I can choose the same form it didn't find from a list... but it inserts it as a new, already created, blank page. Which can be edited, I suppose... but that ain't real elegant.
